请所有参赛选手报名前务必认真阅读《2018飙山越野•龙腾亚丁越野赛暨SKYRUNNING（中国）越野嘉年华参赛声明》，同意并遵守《2018飙山越野•龙腾亚丁越野赛暨SKYRUNNING（中国）越野嘉年华竞赛规程》以及《2018飙山越野•龙腾亚丁越野赛暨首SKYRUNNING（中国）越野嘉年华报名须知》，您提交报名信息后，我们将默认您已阅读并同意以上文件。

作为参赛选手，我、我的监护人、管理人、法定代理人以及任何可能代表我提起赔偿请求或诉讼的人做出以下声明：

1、 我自愿报名参加“2018飙山越野•龙腾亚丁越野赛暨SKYRUNNING（中国）越野嘉年华”（以下简称“比赛”)及一切比赛相关活动，并将如实填写报名相关信息，对所填写内容的真实有效性负责。

2、 我全面理解并同意遵守“2018飙山越野•龙腾亚丁越野赛暨SKYRUNNING（中国）越野嘉年华”组委会及协办机构（以下简称“主办方”）制订的各项规程、规则、规定、要求及采取的措施。

3、 我知悉参加此项比赛对健康状况有特殊要求以及存在的不安全因素，同时对参赛可能存在的各种风险和意外已作出审慎的评估。为参加比赛已做好充分训练和准备并经国家认证的医疗机构体检，确认自身身体和精神健康状况符合参加比赛的各项要求，并愿意按照有关要求接受药物检查。

4、我愿意承担自身的意外风险责任，并确认不会就活动中非组委会原因发生或引发的自身意外、死亡或任何形式的损失向主办方提出赔偿或追究责任。

5、我保证在比赛过程中服从裁判和赛事工作人员的管理和指挥。

6、我同意接受主办方在比赛期间提供的现场急救性质的医务治疗，但在医院救治等发生的相关费用由我自理。

7、我授权主办方无偿使用我的姓名、肖像、声音和其它个人资料用于比赛的组织和推广。

8、我承诺以自己名义报名并参赛，绝不将报名后获得的号码布、芯片等以任何方式转让他人，在比赛期间不损害、更改及遮盖越野赛官方号码布。

9、我已经阅读并知晓“2018飙山越野•龙腾亚丁越野赛暨SKYRUNNING（中国）越野嘉年华”相关保险内容及保险条款，我均予以认可。

10、我或法定代理人已认真阅读并全面理解以上内容，且对上述所有内容予以确认并承担相应的法律责任。

11、我承诺在参赛期间，保护沿途野生动植物和环境，不随意乱丢垃圾。

12、我授权赛事主办方及指定媒体无偿使用本人的肖像、姓名、声音等用于咪咕善跑及赛事相关的宣传与推广。

鉴于本次赛事具有一定竞技风险性，除18周岁以上的完全民事行为能力参赛人外，所有参赛人员均需由其监护人、管理人、法定代理人在本协议签署页签字，以示参赛人的监护人、管理人、法定代理人认可其参赛并自行承担参加比赛所可能存在的风险和责任。

本人及本人的监护人、管理人、法定代理人已全面理解和同意以上内容，保证本人参赛身份/年龄的真实性，此文件由本人及本人的监护人、管理人、法定代理人亲自签署。冒名代签将被视为违约行为，本人、本人的监护人、管理人、法定代理人及冒名者将承担由此引起的全部法律及赔偿责任。

Participant Declaration

All participants, before enrollment, must carefully read the Participation Declaration for China Mountain Trails Yading Skyrunning Festival 2018, and agree with and abide by the Participant Regulations for China Mountain Trails Yading Skyrunning Festival 2018 and Enrollment Notes for China Mountain Trails Yading Skyrunning Festival 2018. After you submit your enrollment information, you will be deemed to have read and agreed on the above documents.

As a participant, I, my guardian, manager, legal representative, and any person who may file a compensation claim or lawsuit on my behalf declare that:

1. I voluntarily enroll in the China Mountain Trails Yading Skyrunning Festival 2018 (herein after referred to as “the race”) and all relevant activities. I will truthfully fill in relevant information and be responsible for authenticity and validity of the content provided.

• 2. I fully understand and agree to abide by the rules, regulations and requirements stipulated and measures adopted by the organizing committee and its co-organizer (herein after referred to as “the host”) of the China Mountain Trails Yading Skyrunning Festival 2018.

3. I am aware of the special health requirements and risk factors of this race and have made a careful assessment of the possible risks and accidents. To participate in this race, I certify that I am fully trained and prepared and that I have been physically examined by a nationally certified medical institution. I confirm that my physical and mental status meets the requirements of the race and I am willing to undergo a drug test in accordance with relevant requirements.

4. I am willing to assume my own risk responsibility and confirm that I will not file any compensation claim against the host or hold the host accountable for any accident, death, or loss in any form caused by or resulting from non-organizing committee factors.

5. I guarantee that I will follow instructions and supervision of the event judges and staff during the race.

6. I agree to accept the on-site first-aid medical treatment provided by the host during the race, but any hospital treatment and other related costs will be at my own expense.

7. I authorize the host to use my name, portrait, voice, and other personal information for the race organization and promotion free of charge. I also authorize the event host and designated media to use my name, portrait, and voice, etc. free of charge for Migu Run and the event-related publicity and promotion.

8. I promise to enroll in and participate in the race myself and will not transfer the bib number, chip, and other items obtained after enrollment to anyone else in any form, and will not damage, change, or cover the official race bib number.

9. I have read, fully know about, and duly acknowledge the related insurance coverage and insurance terms of the China Mountain Trails Yading Skyrunning Festival 2018.

10. I or my legal representative has read and fully understood the above content and duly confirm and assume the corresponding legal responsibility.

11. I promise to protect the wild animals, plants, and the environment along the route during the race, with no littering.

12. This event is competitively risky to some extent; therefore, all participants under18 years of age or those who do not have full capacity for civil conduct, must have their guardians, managers, or legal representatives sign this agreement, to demonstrate that the participants’ guardians, managers, or legal representatives acknowledge that the participate is authorized to enter this race and that there may be potential risks and liabilities for which they must bear responsibility.

I and my guardian, manager, and legal representative fully understand and agree with the above content, and guarantee that my identity/age is authentic. Impersonation of the identification credentials will be considered to be a breach of race rules, and I, my guardian, manager, legal representative, and the impostor will bear all legal and compensation responsibility arising therefrom. This document is personally signed by myself and by my guardian, managers, or legal representatives.
